Step-by-Step Guide: Enrolling at Imagine Madison K - 6th Grade

AGE REQUIREMENTS
We are delighted to accept enrollments for students from Kindergarten through 6th grade, as well as 4-year-old students until September 30th.
For children turning 5 between September 30th and December 31st, we offer early Kindergarten enrollment following a Kindergarten readiness assessment and staff recommendation.
A preschool option is available on our campus for those younger children that would benefit from the gift of additional time before entering the Kindergarten classroom.

RETURNING STUDENTS
Returning students can conveniently complete the re-enrollment process through Final Forms. Have an additional sibling to enroll? Please complete the sibling portion of the of the re-enrollment form to add them, too!

You will need the following items to complete enrollment:
- Child’s Social Security Card
- Birth Certificate
- Residency Verification: Utility Bill or Copy of Lease/Mortgage
- Parent/Guardian State Issued ID
- Record of Current Immunizations
- Last Report Card (optional)
- ETR/IEP Documentation (if applicable)
Re-enrollment Reminder: All returning students will be asked to submit an updated proof of residency and a contact list in Final Forms. Families can also use that re-enrollment form to add a new sibling to our school. Thank you for working with us to ensure your records are up to date!
**Early Kindergarten enrollment evaluation is available for students who turn 5 after September 30th, but before December 31st. Staff will test your child on key Kindergarten readiness criteria and make a recommendation based on the evaluation results.**
